Fees & Insurance
ANNUAL FEES
The membership fee covers your wellness program, AIM4Vitality, and allows the practice to remain small and to guarantee the personalized service every patient deserves. It does not cover any prescriptions, office visits, or other medical services or care that would be covered by your insurance or Medicare. The annual fee only covers the services listed in the Membership Agreement.
The annual fee is $2500 effective April 21,2023. The fee may be paid by check annually or by credit card annually and semi-annually. If paying by credit card, a 3% processing fee will be added.
INSURANCE
All office visits, procedures, and medical care will be billed through your insurance. We will process the claims. Patients will be responsible for any co-pays or deductibles per their individual insurance policies.
Our practice is currently in network with Aetna, BlueCross BlueShield of SC, Cigna, Medicare, TRICARE and United Healthcare. If your insurance company is not included in these, please inquire and we will make every effort to credential with them.
NO INSURANCE? NO PROBLEM!
We are aware that in today's healthcare marketplace, things can be confusing and costs inflated. We are happy to offer patients without insurance discounted prices on services and labs. We make every effort to help all our patients find the least expensive medicines and services.
